Specimen:

Only performed in specialised laboratories.

10 mL EDTA blood for Direct tests (PAIg) and 10 mL plain clot for Indirect. Less for paediatrics by negotiation.

Method:

IF, Flow cytometry, MAIPA, Luminex Xmap technology.

Variously used to demonstrate platelet autoantibodies, drug-dependent antibodies, alloantibodies.

Platelet aggregation for heparin-dependent antibodies (HIT).

Application:

Used in the investigation of immune thrombocytopenias: ITP, drug-induced thrombocytopenia (including HIT), post-transfusion purpura and neonatal thrombocytopenia.

Investigation of refractoriness to platelet transfusion - usually after testing for HLA antibodies and trial of HLA-compatible or HLA-matched platelets.
